    LCMC Health contributions

    • Matching contribution: LCMC Health will match the contributions of full- and part-time employees by 50% up to 4% of pay that is deferred to the Plan.

Disability

Plan basics

Disability coverage replaces part of your income if you’re unable to work due to illness or injury.

    Short-term disability (STD)

    • Coverage begins after 15 days of absence due to an accident or illness, up to 24 weeks 
    • Paid maternity leave: 100% pay for birthing parent for up to 5 weeks (natural delivery) or 7 weeks (C-section), after a 7-day waiting period*

    *Those represented by the union at UMCNO are not eligible for paid maternity leave

    Supplemental short-term disability (STD)

    • Voluntary coverage in addition to the LCMC Health employer paid benefit provides extra protection at 60% on an after-tax basis
    • Helps you avoid using PTO after the 14-day waiting period

    • Coverage begins after 180 days of disability, up to 5 years

    Eligibility and benefits

    • Full-time employees: After one year of service, LCMC Health provides STD and LTD coverage at no cost. Voluntary STD and LTD coverage can be purchased for those with less than one year of service. 
      • Directors and above are eligible for employer-paid STD and LTD benefits on date of hire.  
    • Part-time employees: After one year of service, LCMC Health provides STD at no cost. Voluntary STD coverage can be purchased for those with less than one year of service. Voluntary LTD is available for purchase at the date of hire and during annual enrollment with coverage lasting up to 5 years.

Accidents happen! When unexpected injuries happen, this benefit provides you cash directly — you choose how to use it.

Examples of covered injuries include broken bones, burns, torn ligaments, concussion and joint dislocations.

How critical illness insurance works

When you are diagnosed with a covered condition, submit your claim and you’ll be sent a check. It’s that simple. You can use your money however you choose.

  • Your cash benefit is the amount you select when you enroll in the plan (options are listed in the eligibility and benefits section on this page). 
  • There is no lifetime maximum. If you get sick again with the same or different condition, you’re still covered. There is no total maximum benefit amount to worry about.  
  • Different covered conditions need to be diagnosed at least six months apart. 

Hospital Indemnity Insurance

Plan basics

Hospital stays – whether planned or unexpected – can be expensive. Hospital indemnity pays you a lump-sum cash benefit if you’re admitted to the hospital. You can use this money for deductibles, out-of-pocket expenses, or any other bills.

Examples of covered injuries:

  • Hospital admission: $500 per insured per calendar year
  • Daily hospital confinement: $100 per day, up to a maximum of 365 days per calendar year 
  • Hospital intensive care unit confinement: $200 per day, up to a maximum of 30 days per calendar year 
  • Frequency: Covers one admission per insured per year

How hospital indemnity insurance works

This insurance pays benefits directly to you regardless of another coverage you have.

  • As long as you are an active employee age 18 or older, you and your spouse/partner, and your kids up to age 26 are eligible for coverage. No medical history is required.
